Integra Chiropractic ("we," "us," or "our") respects your privacy. This
Privacy Policy explains what information we collect, how we use it, and
your choices. It applies to our website at https://www.integraccc.com and
our online booking pages at https://integrachiro.janeapp.com.

Note: This policy covers our website and communications. Our Notice of
HIPAA Privacy Practices, which describes how we handle your protected
health information as a healthcare provider, is provided separately at
our clinic and available on request.

1. Information we collect

Information you provide directly:

• Contact information: name, phone number, email address, mailing address
• Appointment and scheduling information
• Health-related information you share through intake forms or messages
• Payment information (processed securely; we do not store full card numbers)

Information collected automatically:

• Device and browser information, pages visited, and general usage data
  (via standard website analytics)

Our online scheduling and patient records are managed through Jane App
(https://jane.app/privacy), which processes patient data on our behalf
under its own privacy commitments.

2. How we use your information

• To schedule, confirm, and remind you of appointments
• To communicate with you about your care, including via SMS text message
  (only if you have opted in — see "Text Messages" below)
• To process payments and manage billing
• To improve our services and website
• To comply with legal obligations

3. Text messages (SMS)

We send appointment reminder text messages **only to patients who have
affirmatively opted in** before the first message is sent. You may opt in
when booking online, on our new patient signup form, through your Jane
patient account, or by giving verbal consent at our front desk.

• Message frequency: at most 2 messages per booked appointment (a reminder
  24 hours before and 2 hours before).
• Message and data rates may apply.
• To opt out at any time, reply STOP to any message or contact us at
  the number below. To get help, reply HELP.

4. How we share information

We do not sell your personal information. We share it only:

• With service providers who help us operate (e.g., Jane App for scheduling
  and reminders, payment processors) — limited to what they need to perform
  their services
• When required by law, or to protect the safety and rights of our patients
  and clinic

5. Your choices

• You may request access to, correction of, or deletion of your personal
  information by contacting us.
• You may opt out of SMS reminders at any time as described above.
• You may disable cookies in your browser settings (some site features may
  not work).

6. Data security

We use reasonable administrative, technical, and physical safeguards to
protect your information. No method of transmission over the internet is
100% secure, but we take the protection of patient information seriously.

7. Changes to this policy

We may update this policy from time to time. The "Last updated" date at
the top will reflect the most recent revision.
