In accordance with Texas House Bill 4224 (codified under Section 181.105 of the Texas Health and Safety Code), this notice provides detailed, step-by-step instructions for our patients to request health records, contact our licensing board, or file a consumer complaint.
1. How to Request Your Health Care Records
Patients have the right to review and obtain copies of their chiropractic and medical records. To request your records from Integra Chiropractic, please follow these steps:
  1. Submit a Written Request: You must provide a signed and dated written request or formal medical records release form.
  2. Delivery Methods: You may drop off your written request in person at our clinic, mail it, or email it to our administrative staff.
  3. Verification & Delivery: Please present or include a copy of a valid government-issued photo ID to safeguard your personal health information. In accordance with Texas law, records will be provided within 15 business days of receiving a valid written request. Standard, reasonable copy or delivery fees may apply.

3. How to File a Consumer Complaint
If you believe your medical records privacy has been violated, or if you have a consumer dispute regarding healthcare services, you may file a formal complaint with the state regulatory agencies:
  • To file a board complaint: Visit the Texas Board of Chiropractic Examiners Online Complaint Portal to access and submit their formal complaint documentation.
  • To file a medical privacy or consumer complaint: You may also contact the Texas Office of the Attorney General — Consumer Protection Division online or via mail under Section 181.103 of the Texas Health and Safety Code.
